Support starts with understanding the person, then agreeing practical steps towards greater stability and independence.

How support is shaped

The starting point is a conversation about whether Amaanah can meet someone’s needs. The detail of support, and whether a placement is suitable, must be agreed individually.

  1. Assessment

    Understand the person’s situation, priorities and support needs through an appropriate referral conversation.

  2. An individual support plan

    Agree what matters, what support is appropriate and what the next steps could be.

  3. Regular support

    Work through the agreed plan with opportunities to raise questions and concerns.

  4. Goals and actions

    Break longer-term ambitions into practical, manageable steps.

  5. Review and progress

    Look at what is working, what has changed and where a different approach is needed.

  6. Move-on and independence

    Keep the next stage in view, including housing options and the support someone may need as they move forward.

Independence is personal

Progress does not look the same for everyone. The aim is to support a person’s next steps, with realistic goals and a plan that can change as their circumstances change.
